In these modules we use the word “parents” which can mean any caregivers important in your life. You may have grandparents, foster parents, or other caregivers that help you in your life.

What is Health Care Transition?

Health care transition means moving from your pediatrician to an adult doctor.

A big part of your health care transition is learning to manage your own health and health care.

You will be learning about the different kinds of doctors you see now or might see as you get older.

It is important to understand what each kind of doctor or health care worker can do for you.
