Before the Visit
Call ahead to make sure they have your medical information from your old doctor.
Make sure you have transportation to the appointment.
Invite a support person to go with you, if you would like.
Make a list of questions you want to ask your new doctor.
Bring a list of your medications.
Bring a list of your allergies, if you have any.
Bring a list of other doctors and specialists you see.
Bring important documents to your visit, like your ID card, driver’s license and insurance card.
Plan to arrive 10 or 15 minutes before your appointment.
