Your Health Care Providers and Medications

One way to manage your health care is to know the names of your doctors and medications. You need to know:

  • which health care workers you see,
  • why you see them,
  • and how to contact them.

It is important to have a list of your doctors’ and specialists’ names.

If you take any medications, you should know:

  • the name of each medication
  • the dose of each medication
  • what it looks like
  • how you take it
  • when and how often you take it
  • and why you take it.
